Incident Follow-Up – Document Generation Outage (6/16)
Last week, we resolved a major issue where note generation was stuck in processing. The root cause involved a breakdown in how our summary service interacted with BullMQ and Redis.
We resolved it by rebuilding Redis, restoring all delayed notes, and deploying fixes to improve system resilience.
We’re increasing observability, improving our queueing architecture, and putting better alerts in place to catch issues faster next time.
